Enrollment Procedure

FOR FIRST YEAR ENROLEES


I. ACCOUNTING OFFICE - SLCM 8th Floor, Room 802

1)Obtain and sign the official registration form from the accounting office and settle the required fees

2)Obtain a request for x-ray from the Accounting Office - Cashier (Room 802) and settle the required Medical and Dental Fee

3)For Foreign

An Institutional Development Fund Fee (IDFF) and Certificate of Eligibility Stamp Fee are required for Foreign and Foreign Graduate Students. This is non-refundable.

II. MEDICAL AND DENTAL EXAMINATION PROCEDURE

A. CATHEDRAL HEIGHTS BUILDING COMPLEX (CHBC)

1. Present the request for X-ray Examination and obtain an x-ray permit from the Cashier (Out-Patient Services, North Tower, Ground Floor).
2. Present permit to the OPD x-ray department and obtain an x-ray examination.
3. Pick-up x-ray results on the scheduled date, then proceed to

B. HEALTH CLINIC - SLCM 8th Floor, Room 808

1. Submit the X-ray examination result to the Nurse and get a schedule for a medical and dental exam.
2. After completing the scheduled medical and dental exam, make sure that the medical and dental forms have been duly signed by the doctor and return it back to the Nurse. This is the final step for the medical and dental clearance.

III. REGISTRAR'S OFFICE - SLCM 8TH FLOOR, Room 804

1. Present the stamped PAID and validated Registration Form.

2. INITIAL REQUIREMENT

Present the following Original copies of credentials for enrollment:

a) NMAT result;

b) Birth Certificate (NSO / Authenticated)

c) Income Tax Return (ITR) - For Scholars Only

3. COMPLETION REQUIREMENT

(The following documents to complete Registration may be submitted on or before May 31, 2015)

a) Transfer Credentials or Honorable Dismissal;

b) Official Transcript of Records with remark "GRADUATED";

c) Fill up WAIVER

Once all your documents are complete including your medical, dental evaluation, and your registration form is signed by the Registrar, yo are officially enrolled.

 

FOR OLD STUDENTS

A. ACCOUNTING OFFICE

Obtain the official registration form and the request for x-ray examination from the Accounting office and settle the required fees.

B. CATHEDRAL HEIGHTS BUILDING COMPLEX (CHBC)

1. Present the request for X-ray Examination and obtain an x-ray permit from the Cashier (Out-Patient Services, North Tower, Ground Floor).
2. Present permit to the OPD x-ray department and obtain an x-ray examination.
3. Pick-up x-ray results on the scheduled date, then submit x-ray results to the Nurse at the Health Clinic to complete your medical requirements.

C. REGISTRAR'S OFFICE

1. Present the stamped "PAID" and validated Registration Form.
2. Once all your documents are complete, including your medical requirements, your registration form will be signed by the Registrar. Then you are officially enrolled.
